2018: hysterectomy and the first weight change

After a decade of endometriosis and multiple surgeries, Dunham had a hysterectomy in early 2018 at 31. She wrote about it in Vogue, including the grief of knowing she would not carry a pregnancy. Her weight rose afterwards, partly from the hormonal changes and partly, as she has said, because she stopped treating thinness as a goal that illness had made unreachable anyway. The same year she quit a benzodiazepine dependency and has been sober since; in July 2025 she told CNN that staying sober was “the right thing for me.”

2021: the wedding and the comments

Dunham met Luis Felber in London in early 2021 and married him that September, in a dress by Christopher Kane. She posted the photos on Instagram, and the comments underneath turned into a referendum on her weight. Her reply, in a long post, has become the quote most associated with her on the subject: she said she was neither thin nor fat, that she had been both and been shamed for both, and that her body at that moment was the one that had survived chronic illness and addiction. She asked followers to stop treating any body as a “before” picture.

2022: the Guardian interview

Promoting Catherine Called Birdy in 2022, Dunham told The Guardian that watching herself on screen in the early Girls era had been painful because she still believed the standards she was being judged by. By 2022 she said she had stopped, not because the comments ended but because she decided they were not information she needed.

2024: “not up for having my body dissected again”

In 2024 Dunham announced Too Much, a Netflix comedy about an American woman who moves to London after a breakup, loosely based on her own life. She cast Megan Stalter as the lead and gave herself a small supporting role. Asked by The New Yorker why she would not star in her own story, she said: “I was not willing to have another experience like what I’d experienced around Girls at this point in my life. Physically, I was just not up for having my body dissected again.”

It is a striking admission from someone who built a career on self-exposure, and it is the clearest measure of what a decade of body commentary cost. The show premiered on July 10, 2025, and the reviews, for once, were about the writing. In November 2025 Dunham confirmed there would be no second season, saying she had always intended it as a limited series.

2025: health in her own words

While promoting Too Much, Dunham spoke about living with Ehlers-Danlos syndrome, a connective-tissue disorder, alongside endometriosis and fibromyalgia. She has described weight fluctuation as a side effect of chronic illness and its treatment, not a project she is working on in either direction, and has called the public’s fixation on the shape of her body “insane.”
